COMPREHENSIVE ASSESSMENT
AND TREATMENT PROGRAM FOR ANXIETY


Much of the work in helping you overcome your anxiety disorder relies on well-established principles and techniques of cognitive-behavioral therapy. Basic strategies such as abdominal breathing, muscle relaxation training, replacing catastrophic “what-if” thoughts with constructive self-talk, or gradually facing a phobia through incremental exposure, are the foundation of effective treatment. These are the strategies that are emphasized in the early part of therapy. Prescription medications may also be recommended when your condition is seriously disrupting your life or of long duration. In general, cognitive-behavioral therapy and medications are effective. These interventions help you to: 1) relax your nervous system and body, 2) retrain your mind to think constructively rather than fearfully, and 3) face situations you’ve been avoiding. All three of these are essential for your recovery. If you do the homework assignments given and practice cognitive-behavioral strategies regularly, your condition will definitely improve.

Sometimes, however, more than cognitive-behavioral therapy and medication are needed to ensure the highest possible degree of recovery. There are several other areas--outside the scope of standard treatments--that may impact your anxiety condition. For example, your life may be so complicated that you end up with preciously little down time to rest and rejuvenate yourself. Learning time management and other strategies to simplify your life may be needed. Or your diet may be aggravating your stress level, and thus your anxiety. While you may know to cut down on caffeine and sugar, even before coming in for therapy, you may be less aware of subtler dimensions of diet that can affect anxiety, such as food allergies, carbohydrate to protein ratio, candida, thyroid problems, mineral deficiencies, or heavy metal toxicity. Other areas that may influence your anxiety issues include your marital and family relationships, having a sense of purpose about your life, and your spiritual orientation and convictions. So, beyond the help you receive from cognitive behavioral therapy and medication, you may, for example, need to simplify your life, resolve marital difficulties, have a creative hobby or project that gives your life meaning, and/or deepen your trust and conviction in a Higher Power. All of these areas can affect your overall sense of well-being and satisfaction with your life. As such, they are likely to significantly impact your anxiety symptoms, whether panic attacks, worries, or phobias.

The Comprehensive Assessment and Treatment Program includes the Basic Program plus expanded focus on several additional areas. It begins with your completing a series of questionnaires. You will receive a questionnaire for each of the following seven areas. You can then review and decide which questionnaires you wish to complete (all eight are recommended but not necessary), and then return them to Dr. Bourne.

3. Simplify Your Life
4. Core Beliefs/Personality Traits
5. Intimate Relationships
6. Life Purpose
7. Spirituality

For each questionnaire that you complete, we will spend one session together going over your results and discussing corrections and changes you might make in your life for the better. The nutrition and diet questionnaire may involve two sessions. The purpose of each questionnaire is to help you move beyond basic cognitive behavioral treatment interventions and address the full spectrum of issues that are relevant to helping you achieve the highest level of recovery.


Cost

The cost of the program is $25 per completed questionnaire plus the cost of a one hour session to discuss that questionnaire. So if you decide to complete four questionnaires, you will be charged $100 for the time it takes me to read and evaluate your responses to all four. The subsequent telephone sessions we have together to go over your results for each questionnaire will be charged at my regular rate of $80 per one hour session. In brief, your cost will be $105 per questionnaire and accompanying session. Total cost will depend on the number of questionnaires you decide to complete.

When you have completed your questionnaires, please return them to me at the address below. Please enclose payment by check @ $25 per completed questionnaire. Upon receiving the questionnaires, I’ll review them and then call you to schedule sessions for us to discuss them.
